2004-2007 REAL CHOICE SYSTEMS CHANGE GRANT-REBALANCING INITIATIVE The project is working on a plan for rebalancing of funds between long-term care (nursing home/institutional) services and those services provided in home or community settings. The project is also looking at developing a new system for providing a single point of entry for services for elderly and persons with disabilities who are considering long-term care and home and community-based services. This type of service, commonly referred to as an Aging and Disability Resource Center (ADRC), is common in many states but will be new to North Dakota. The project involves bringing together representatives from public and private organizations that play a role or are interested in assuring that North Dakota elderly and persons with disabilities have options and access to the continuum of long-term care services in North Dakota. The Consensus Council has provided meeting facilitation to the project participants since the beginning of the initiative in 2004, through its completion in 2007. |
